• How to obtain a Milpitas RACES Mission/Activation Number  (includes form and log).

• Requesting Mutual Aid - This is an MS Word form for the city to fill out to request RACES Mutual Aid from the Santa Clara County Office of Emergency Services.  Complete the form and have it signed/approved by an appropriate City Official.

  This form is also available to send via Packet within PacForms on the Packet Laptop at the EOC.

  After receiving approval for the Mutual Aid Request notify the County DEC/CRO of the request via voice radio or telephone and send the form to county OES via Fax, email, or Packet when available.  Copies should also be provided to the Milpitas CRO and the Milpitas Office of Emergency Services.
