Mission / Activation Numbers


It is the respopnsibility of RACES leadership to ensure a Mission Number (Activation #) is created and approved for all Activations of RACES personnel.  Additionally the names of each RACES Responder must be recorded using an ICS-211 form.

Excerpted from City of Milpitas Standard Operating Procedure 4.6:

    All RACES activations will be assigned a Mission Number in the format MLP-yy-nnn where yy is the year and nn is a serial number begining with 001 at the start of each year.  RACES training activities will have a T suffix.  Examples: MLP-02-003 for the third activation of 2002 or MLP-02-004T for the fourth activation which was a training mission.

To obtain a Mission (Activation) Number complete the Milpitas RACES Activation Request form. Determine the next Mission/Activation number using the log file linked below.  Have the form signed by the Competent Authority authorizing the activation.  Forward a copy of the form to the Office of Emergency Services and the Chief Radio Officer at your earliest convenience. This form can also be found on the RACES laptop at the EOC.

If the city activates RACES for something other that a training or public service event the county DEC/CRO needs to be notified of that fact.


Milpitas RACES Activation Number Log (Excel File)
This form is for the CRO and DCROs to log the Mission/Activation Number for each RACES Activation.  The form cannot be saved back to the Web site, so after filling it out, save it, and E-Mail it to Tim Howard at [email protected] and the form will be replaced.  A copy of this log will be kept in the RACES Reference Binder at the EOC but will only be updated each time an activity takes place there, so it may not be the most current version.
