You're about to access the membership application for The QSY Society. Please note the following simple directions regarding the form:
1) Fill in the boxes - required fields are marked with an asterisk (*). You can tab between fields to quickly move through the form.
2) When you're done filling out the form, print it using your browser's "Print" button.
3) Sign and date the form, then either mail it to the address at the top of the page or bring it to the next scheduled meeting.
That's it! No dues or initiation fee are required. Consider yourself a member and come to the next meeting so we can meet you in person.
