Wilco,

When you asked how are they made, are you asking how are
they simulated or how they are measured?

WRT how they are interpreted, I think that the various
artifacts within the eye can each be caused by many
artifacts. Interpreting the artifacts takes experience
with knowledge of the circuit and if its a measured
eye diagram you also have to know your test equipment well.
