I guess that it is now OK to plug your or someone else's book.
A new book entitled:

High-Speed Digital System Design - A Handbook of Interconnect
Theory and Design Practices by Stephen Hall, et. al. -
published by John Wiley & Sons, Inc. (Wiley Interscience)

has been out for a couple of months now. The book is quite comprehensive
and covers many of the 'modern' signal integrity issues.
