Another possibility would be to use AVX Low Pass Filter Arrays, see
http://www.avxcorp.com/pdf/lpfarray.pdf These put four isolated
resistors, each with a distributed capacitor, into a 1206 (0.128" x 0.063",
3.25mm x 1.60mm) surface-mount package. The datasheet says that resistor values
from 10- to 1000-ohms +/-10% and capacitor values from 10 to 100pF +/-20% will
be available. This is a brand new line for AVX, so they don't have standard
values available yet, but I received some Z3F43Y120M101MAT samples with
100-ohm resistors and 12pF capacitors from them. The Product Marketing Manager
