I just got a copy of a new interconnect theory and design book which may interest you. In addition to the normal transmission line theory, it describes the design of an interconnect including buffer modeling, timing analysis, non-ideal effects (return path and frequency dependent loss), as well as modern design methodologies and it covers a lot of other signal integrity issues. There is also a chapter on measurement techniques (TDR and network analyzer).
