Anyone wishing to book a table should fill out an application form and return it with the required payment by mailing it to Les Parker VK4ZLP at 22 Auckland Street Wishart 4122

To obtain an application form simply select APPLICATION FORM

Print the blank form, then after filling it out, return it with the necessary payment.
