Notes for Registration and Reservation Form.

The following numbered paragraphs correspond with those on the R&R Form, and contain notes to help you complete the form.
1. The leading attendee should complete this section. I.e. Group leader, or leading partner in the case of a couple. The badge name is the name or call-sign you wish to be put on your Convention ID Badge.
2. Please complete the postal address of the Leader.
3. Please give the contact details of the Leader
4. Please give the names and badge names of all accompanying persons.
5. Please fill in the total number of persons in your party aged over 21 years. The Convention Registration Fee is 85 USD (RM 323.00) per person. This covers lunches, convention dinners, entertainment, convention activities, sight-seeing, prizes etc. The total registration fee for such persons can be calculated by multiplying the number of persons by 85 USD (RM 323.00).
6. Please fill in the total number of persons in your party aged below 21 years: The total registration fee can be calculated by multiplying the number of such persons by 55 USD (RM 209.00)
7. SARS will arrange to meet delegates arriving at Kinabalu International airport (BKI), and will arrange the most appropriate transportation by road to the Pacific Sutera Hotel, Sutera Harbour.
8. Please give your flight/arrival details if possible. If not please mark "will forward later".
Note: To qualify for preferential airfares provided by the Official Airline, MAS, please contact your nearest Malaysia Airline’s Office in your area, quoting SEANET2001 Convention code G*QDAGMD. If there isn’t a MAS office in your area, please send me this Registration form duly filled and we will forward your contact to MAS who will contact you directly.
This preferential arrangement is not applicable for travel from domestic cities, Singapore and Brunei.
9. Accommodation can be arranged and reserved directly with The Pacific Sutera Hotel, via their Web site for reservation:  http://www.suteraharbour.com/reservations/index.html or by Email: [email protected] . Please mention that you are attending the SEANET 2001 Convention to qualify for the special SEANET2001 price of RM180.00 Nett per single/double room per night with ABF.

Tel : +6088-318 888  Fax : +6088-317 777   It should be mentioned that The Pacific Sutera Hotel is normally fully booked during November (High Season), but is reserving a large number of rooms for the convention. Once the rooms are booked, an alternative hotel may have to be used.

To guarantee your room booking in the convention hotel it is preferable to book as soon as possible, and either make a 10% deposit or quote a credit card number to the hotel. 10. Please indicate your accommodation requirements. Note that the prices are nett, and are specially discounted for the convention. ABF is American Breakfast, which in fact is an International Buffet Breakfast at RM180.00 per room ( single / Double).  *Please indicate your arrival and departure dates, and # of nights.
11. The Pacific Sutera Hotel is offering a very good value package, and it is obviously more convenient to be located in the Convention Hotel, but if anyone would like help in locating alternative accommodation in Kota Kinabalu City, SARS will be pleased to do so. Please indicate if help is required.
