FISTS FORMS
Several FISTS members requested a copyable logbook that is suitable for awards tracking. I created a FISTS Logbook Cover Page and a FISTS log page. You may download either one by clicking on the appropriate link below. You must have Adobe Acrobat Reader to view and print these pages.
The quality of the above image is poor compared to the actual artwork. If you would like a "camera ready" copy mailed to you, please send SASE to N9BOR. For unfolded copy, envelope must be larger than 8 1/2" x 11".
FISTS Award Forms and Worksheets
I also created a FISTS "Worked All States" application form. This form is useful for keeping track of your contacts for this award. Click here to download a copy in Adobe Acrobat Format.

A MS Excel worksheet file is also available for tracking and submitting the Century Awards, WAS and Ragchewer Award.
