New Jersey utilizes a multi-tier EMS System. Basic Life Support (BLS) ambulances are operated by municipal or city government, private run volunteer squads or private corporations. Advanced Life Support (ALS) is provided by hospital based Mobile Intetnsive Care Unit (MICU). A patient requiring ALS would require both agencies and at least 2 vehicles dispatched. If necessary the paramedic would ride in the BLS ambulance and the ALS vehicle would follow behind to the hospital emergency department. BLS EMT's in New Jersey do not intubate or start IVs.

Criteria for BLS or simultaneous BLS and ALS dispatch are written by the New Jersey Office of Emergency Telecommunication Services (OETS). Guidecards or computer assisted screens would prompt the emergency medical call taker with appropriate questions and and guidelines to make a dispatch determination. New Jersey EMT-Basic program utilizes a 120 hour course cirruiculum, 110 hours classroom + 10 hours E.R.

New Jersey's aero-medical evacuation is run by the New Jersey State Police JemSTAR (Jersey Emergency Medical Shock Trauma Air Rescue) program. The aviation unit and medevac are part of the NJSP Emergency Management Bureau. There are two main helicopters for the state. One serves the northern portion of the state NorthSTAR N5NJ and the other in the Southern portion of the state SouthSTAR N9NJ. Both helicopters are Sikorsky S-76B aircraft. They are backed up by the NJSP Aviation unit N1NJ (Governor's ship also a Sikorsky) and Bell Jet Rangers when main aircraft are out of service. In New Jersey there is no charge for med-evac service only for the hospital supplied flight-medic/flight-nurse on board. There is a $1.00 surcharge on motor vehicle registrations that fund the JemSTAR program.
