Call Sign License Plates
If you would like to obtain "Call Plates" for your CA registered vehicle, click on the "Call Plate" icon below and complete CA-DMV Form REG 17A; this form can be completed on-line. It can also be used to transfer your current "Call Plates" to another vehicle, see special note below.

The process is as follows:
Complete Parts 1, 2, and 5 only!
Step 1:
Check the "Original" box in the top left corner.
Step 2:
Be sure you put the same name and address that appears on your current registration. It is also important that the name on the registration of the vehicle you want "Call Plates" for, matches the name on your current Amateur Radio License.
Step 3:
-Check type of vehicle. Yes, you can even get "Call Plates" for your Motorcycle.
-Fill in CURRENT plate number (not your Call Sign) and VIN.
-Check "Amateur Radio License" box and print your Call Sign in block letters.
Step 4:
Sign and Date the form and be sure to include your daytime phone number.
Step 5:
The last step is to mail the completed application to the CA-DMV, don't bother standing in line at the local DMV office. I would send it Certified, Return Receipt Requested to the following address:

P.O. Box 932345
Sacramento, CA 94223-3450
You should have included the following:
-Completed CA-DMV Form REG 17A
-A check or money order made out to CA-DMV in the amount of $21.00
-A photocopy of your current Amateur Radio License.
That is all there is to it, once the application is received at the CA-DMV in Sacramento it should take 6-8 weeks for you to receive your new "Call Plates" in the mail. I would recommend that after you install the "Call Plates" on your vehicle, turn your old plates into the local DMV office. Again, don't bother standing in line, all you have to do is drop them off at the counter.
SPECIAL NOTE:
If you need to transfer your current "Call Plates" to another vehicle you must use CA-DMV Form REG 17A. The form is completed as above, except the "REASSIGN, HOLD, RELEASE" box must be checked instead of the "ORIGINAL" box and Part 3 must be completed as well; there will also be a $12.00 transfer fee. Unfortunately, you will have to stand in line at the local DMV office. Bring all the paperwork (i.e. registrations, bills of sale, plates, etc...) for both vehicles to complete the transfer.
