Here are some pictures of the Quad.
Here she is up on the 40 foot tall self supporting tower.  3 elements on 20, 17, and 15 Meters and 5 elements on 12 and 10 Meters. 
It's off center, with 8 feet of boom on the short side and 12 feet of boom on the longer side (where the driven elements are located).

This picture shows the construction phase, the tower is tilted from the base.  A wood support brace was needed to keep the boom horizontal while the elements were slid onto the 3 inch diameter boom.  The spreaders are fiberglass poles.  That's me staring at the ground!
In this view you can see the wood "cradle" that was needed to support the tower at a good working height.  Most of the hardware came from Cubex, I combined two of their quads to make this monster. No, that's not my trailer!
I used five separate feedlines and no matching transformers, just a few ferrite beads at each feed point.  If I find or build a relay box that I can trust, I'll consider reducing the number of coax runs in the future.  The dimensions of elements came from the June, 98 CQ magazine article by K0SR.
